FINAL REPORT GUIDELINES
2002 TECHNOLOGY TRANSFER AWARDS
July 2002
Your CASI Technology Transfer award ends on August 31, 2002. The final report
should be produced in a technical report format as described below. Because
the final reports are published as CASI Technical Reports, it is important that
these guidelines be followed.
The final report is due November 1, 2002, approximately 3 weeks after the
Research Symposium and two months after the end of the grant period. It provides
sufficient time for the report's clearance through the Collaborating Company
as required under "Reporting
Safeguards" in the Terms and Conditions. The Intellectual Property Disclosure
Form (Attachment 1) must
be attached to the Final Report at the time of submittal.

- Send the ORIGINAL manuscript
- Do NOT staple
- Page One should begin with the body of the report.

Title Page
The title page must include the following information (see Attachment 2 for sample):
- The words "Final Report"
- The words "Colorado Advanced Software Institute"
- The project title
- The name(s) of the principal investigator(s)
- The name of the graduate student
- The name of the Collaborating Company
Please note that the title page should not include a date. The publication date will appear on the reverse side of the technical report cover along with other publication information.
Release Statement Page
The release statement page will be printed on the reverse side of the title page. It must include the following information (see Attachment 3 for sample):
- Project Title
- Principal Investigator
- University
- Collaborating Company
- Collaborating Company Representative
- The following statement:
"As authorized representative of the collaborating company, I have reviewed this report and approve it for release to the Colorado Advanced Software Institute."
- The above statement must be followed by a signature and date line signed by the company representative.
Table of Contents
The pages of the Table of Contents should be numbered with small Roman numerals (i, ii, iii, etc.). Page numbering with Arabic numbers (1, 2, 3, etc.) should begin with the body of the report.
Abstract Page
Body of the Report
The body of the report must be prepared as a single-sided manuscript in standard technical report format, page-numbered with Arabic numerals beginning with Page 1.
Please note the following:
- Send an original, camera-ready document with artwork clear enough for good reproduction. Do not staple the document.
- Format the pages for front-to-back reproduction with a 1 1/2" left-hand margin to allow for binding. Watch especially the placement of page numbers.
- The report should not include source code listings but should indicate how code might be obtained.
- The body of the report is not limited to but must include the following sections:
- Objectives
- Approach
Note: If either objectives or approach changed, this must be clearly described with the reasons for such change. In case of a major change, the revised proposal (as approved by the CASI director) must accompany the report.
- Results
- Evaluation
- Technology Transfer section that describes the technology exchange going in both directions between you and the Collaborating Company. A section written by the collaborating company must describe the technology transfer from their point of view.
- Networking (conferences and other professional meetings) - include those attended and those expected to attend through June 30, 2001
- Publications (list published, accepted and submitted papers)
- Funding attributable to CASI support. Please report other proposals (title, funding source, and
amount) related to the CASI project (a) under preparation; (b) under review; (c) submitted and declined
for funding; and (d) submitted and awarded.
